Common use
Imitrex is indicated for the acute treatment of migraine attacks with or without aura in adults. It is most effective when taken at the very first sign of migraine pain. It is not to be used to prevent migraines or to treat common tension-type headaches. The choice of formulation (tablet, injection, nasal spray) is determined by the physician based on the severity of the attacks, the speed of onset required, and the presence of gastrointestinal symptoms like nausea and vomiting that may hinder oral absorption.
Dosage and direction
Tablets: The recommended initial dose is 25 mg, 50 mg, or 100 mg. If symptoms persist or return after initial relief, a second dose may be taken after at least 2 hours, not to exceed 200 mg in a 24-hour period. Injection: The recommended dose is a single 6 mg subcutaneous injection. If symptoms persist or return, a second 6 mg injection may be administered after at least 1 hour, not to exceed two 6 mg injections in a 24-hour period. Nasal Spray: A single spray of 5 mg, 10 mg, or 20 mg into one nostril. If symptoms persist or return, a second dose may be administered after at least 2 hours, not to exceed 40 mg in a 24-hour period. The dosage must be individualized by a healthcare provider. The patient should not exceed the maximum daily dosage for their prescribed formulation.
Precautions
- Use Imitrex only for a condition for which it is prescribed. Do not share it with others.
- This medicine may cause dizziness, drowsiness, or weakness. Do not drive, operate machinery, or perform hazardous tasks until you know how Imitrex affects you.
- Imitrex should not be used in patients with risk factors for coronary artery disease (e.g., hypertension, high cholesterol, diabetes, smoking, obesity, strong family history) unless a cardiovascular evaluation provides satisfactory evidence that the patient is free of underlying cardiovascular disease.
- Use with caution in patients with hepatic impairment, as dosage adjustment may be necessary.
- Patients should be informed of the risk of medication-overuse headache (rebound headache) from frequent use of any acute migraine medication.
Contraindications
- History of ischemic heart disease (e.g., angina pectoris, history of myocardial infarction, documented silent ischemia).
- History of coronary artery vasospasm, including Prinzmetal’s angina.
- Uncontrolled hypertension.
- History of stroke or transient ischemic attack (TIA).
- Peripheral vascular disease.
- Ischemic bowel disease.
- Hemiplegic or basilar migraine.
- Severe hepatic impairment.
- Hypersensitivity to sumatriptan or any component of the formulation.
- Concurrent administration or within 24 hours of another 5-HT1 agonist (triptan) or an ergotamine-containing or ergot-type medication (e.g., dihydroergotamine, methysergide).
Possible side effect
Common side effects include:
- Sensations of pain, pressure, tightness, or warmth in the chest, throat, neck, or jaw (typically non-cardiac in origin but must be evaluated).
- Dizziness, vertigo.
- Drowsiness, fatigue.
- Flushing (feeling of warmth).
- Injection site reactions (pain, redness).
- Unusual taste following nasal spray administration.
- Nausea and vomiting.
Serious side effects (seek immediate medical attention):
- Signs of a heart attack: chest pain, shortness of breath, irregular heartbeat, pain spreading to the jaw or shoulder.
- Signs of a stroke: sudden numbness or weakness, confusion, vision or speech problems, severe headache.
- Signs of decreased blood flow to the bowels: sudden or severe stomach pain, bloody diarrhea.
- Signs of an allergic reaction: hives, difficulty breathing, swelling of the face, lips, tongue, or throat.
- Seizures.
- Serotonin syndrome: agitation, hallucinations, fever, fast heart rate, muscle stiffness, twitching, loss of coordination, nausea, vomiting, diarrhea.
Drug interaction
Imitrex has significant and potentially dangerous interactions with several drug classes:
- Monoamine Oxidase-A Inhibitors (MAOIs): Contraindicated within 2 weeks of taking an MAOI.
- Other Triptans and Ergot Derivatives: Contraindicated within 24 hours of each other due to increased risk of prolonged vasospastic reactions.
- Selective Serotonin Reuptake Inhibitors (SSRIs) and Serotonin-Norepinephrine Reuptake Inhibitors (SNRIs): Concomitant use may increase the risk of serotonin syndrome.
- Cimetidine: May increase sumatriptan plasma concentration.
Missed dose
Imitrex is not used on a scheduled basis; it is used only to treat an acute migraine attack as it occurs. Therefore, the concept of a “missed dose” does not apply. Do not take extra medicine to make up for a missed attack.
Overdose
In cases of overdose, symptoms may include hypertension, more pronounced versions of the drug’s known side effects, and cardiovascular events. There is no specific antidote for sumatriptan overdose. Treatment consists of supportive measures, including continuous ECG monitoring for at least 12 hours, and management of hypertension if present. The patient must be taken to a hospital for immediate medical attention.
Storage
- Store at room temperature between 20°C to 25°C (68°F to 77°F). Excursions permitted between 15°C and 30°C (59°F and 86°F).
- Protect from light and moisture.
- Do not freeze.
- Keep the injection unit in its sealed tray until ready to use.
- Keep all medications out of the reach of children and pets.
